Lisa Carey, MD, FASCO, ScM, offers a clinically grounded tour of how genomic and immune biomarkers are transforming breast cancer care, from early-stage treatment decisions to metastatic disease management. Dr. Lisa Carey reviews the dramatic improvements in survival driven by targeted therapies, the biology underlying intrinsic tumor and immune subtypes, and the strengths and limitations of major biomarker assays in ER-positive, HER2-positive, and triple-negative breast cancers. Using evidence from large genomic datasets and key trials—including CALGB 40601, CALGB 40603, HER2DX development, and ongoing studies such as HARMONY—she illustrates how tumor subtype, immune activation, and molecular heterogeneity influence response to therapy, recurrence risk, and treatment tailoring. The session highlights where biomarkers work well, where they fall short, and what’s coming next, providing clinicians with a practical framework for matching the right patient to the right treatment at the right time.

  • Identify current and emerging predictive and prognostic biomarkers in nonmetastatic breast cancer.
