Site-Coordinator Upload

For group viewing of a UNCLCN Live Webinar, we ask that designated site-coordinators download and print a roster form then upload the completed form on this page after the webinar.

Once received, a UNCLCN staff member will provide individual instructions to each participant to complete. Rosters received after 30 days may not be accepted and subject for review. All viewers must attest to viewing to at least 53 minutes of the webinar.
